Palliative Care

hospital buildingOur hospital based palliative care program is provided by a team of specialists that include:
  • A Board Certified Palliative Care Physician
  • Palliative Care Nurse
  • Registered Dietitian
  • Pastoral Care
  • Social Worker
  • Pharmacists
  • Case Management

This team works together with your primary doctor/hospitalist to provide an extra layer of support for your care during your hospital stay. The main goal of the team is to help you have more control over your care, concentrating on the problems that affect you the most. Working together with you and your doctor, the palliative care team provides:

  • Time for close communication.
  • Expert management of pain and other symptoms.
  • Help navigating the healthcare system.
  • Guidance with difficult and complex treatment choices.
  • Emotional and spiritual support for you and your family.

One of the goals of palliative care is to provide a smooth transition for the patient between the hospital and appropriate services, such as hospice, home care or nursing home.
