I Need a Patient Portal Account

If you have been a patient at a HealthAlliance Hospital (Mary’s Avenue Campus, Broadway Campus, or Margaretville Hospital) from 5/6/2013 to date and are 13 years of age or older, then you are eligible to participate in our patient portal.

You will need access to a computer connected to the internet and an up-to-date browser such as Internet Explorer. You will also need a valid e-mail address which will be used to send you the initial patient portal registration invite and to notify you of any new messages in your portal account. Since it is treated with the same privacy and care as your health records, we recommend that you use an e-mail address to which only you have access to.

To request access to our patient portal, click on the link provided on this page to download our Patient Portal Terms of Use and Authorization for Release forms.

Complete all forms with the appropriate signatures and mail to:

Once the information provided on the forms has been verified and set up in our system, you will then receive an e-mail invite from HealthAlliance that contains a specific website link to our Patient Portal for you to use to register. Our Patient Portal brochure will further explain about this registration process.
