How do I sign up for the HealthAlliance Patient Portal?

If you…

  • Have not received your e-mail invite within 36 hours of discharge from the hospital;

  • Experience problems logging in or using the Patient Portal;

  • Have trouble changing your password or any other technical issues; or

  • Believe that there has been unauthorized access to your patient portal;

If you…

  • Have questions about how to sign-up for the HealthAlliance Patient Portal;

  • Need to request a copy of your hospital medical record (Release of Information);

  • Want to change your status and not have future hospital visit health information sent to the portal; or

  • Suspect that someone who should not have seen or gotten access to information about you, has done so;
